Jake Hernandez, PMHNP-BC
I grew up in Queens, NY, raised by parents from the Dominican Republic. Mental health wasn't a topic discussed in my home until, at 16, I came out as gay. That moment marked a turning point, and therapy became a vital safe space for both me and my mother. It also sparked my passion for pursuing a career in psychiatry.
I’m a board-certified psychiatric-mental health nurse practitioner (PMHNP) with licenses in both New York and Florida. I earned my bachelor's degrees in nursing and psychology from New York University, followed by a master's in psychiatric-mental health nursing.
Before becoming a PMHNP, I worked as a psychiatric registered nurse at an inpatient psychiatric facility for college students in New York City.
I founded ZenzyTalk to bridge gaps in mental health care and bring a sense of relatability to the patient-clinician relationship. My approach combines medication management with adjunctive psychotherapy, focusing on adults with conditions such as depression, anxiety, ADHD, and more. I take an integrative and holistic approach to care.
Currently, I live in Orlando, FL, where I can often be found at a local coffee shop on my days off. I’m an avid traveler, constantly adding new destinations to my list.

Your initial evaluation is a comprehensive assessment that reviews your symptoms, medical and psychiatric history, lifestyle factors, treatment goals, and previous treatment experiences. Together, we'll develop a personalized care plan tailored to your needs.
Yes. ADHD treatment may include comprehensive evaluation, cognitive testing when appropriate, medication management, and lifestyle-based interventions. Treatment recommendations are individualized based on clinical assessment and applicable state and federal regulations.
We provide psychiatric care for adults experiencing anxiety, depression, ADHD, OCD, insomnia, mood disorders, trauma-related concerns, PMDD, and other mental health conditions.
Yes. In-person appointments are available in Central Florida, and I offer quarterly in-person appointment days in New York City. While most visits are conducted virtually, certain treatment plans may require or benefit from in-person follow-up.
We require at least 24 hours' notice for cancellations or rescheduling of appointments. Failure to provide adequate notice may result in a fee.
We are in-network with Aetna, Anthem/Empire BCBS (NY), Carelon, Cigna, Florida Blue, and BCBS Massachusetts. Insurance participation may change over time, so we encourage you to verify your benefits prior to scheduling.
